Quick answer
Descale a tankless unit annually in moderately hard water and more often above about 10 grains per gallon. At 15 grains per gallon with a 140 F setpoint the interval drops to roughly 7 months, because scale deposition rises with both hardness and temperature.
Result
Scale costs efficiency long before it causes a fault: a scaled heat exchanger transfers less heat for the same gas, so the unit runs longer for the same hot water. If you are choosing whether to treat the water rather than how often to flush it, that is a water treatment question and homewatercalc.com covers it.
The formula
Interval = base interval for the hardness band, reduced by a quarter for a setpoint at or above 140 F and again for heavy use. Grains per gallon = parts per million / 17.1.
THIS IS A CONVENTION and it is worth being blunt about that. No test procedure publishes a descaling interval. What is published is that calcium carbonate deposition rises sharply with both hardness and surface temperature. Manufacturers say "annually" and then say "more often in hard water" without defining hard, which is the gap this table fills.
The figures above are computed with these exact inputs: Water hardness 10 gpg, Outlet setpoint 120 F, Heavy daily use no.
Some of these are published test procedures or code tables and some are planning conventions. They are not the same kind of number, so each one is labeled.
| Assumption | Kind | Where it comes from |
|---|---|---|
| Descaling interval against hardness | Convention | Descaling interval against hardness. No test procedure publishes one, and manufacturers say annually without defining hard water |

Right angle adapters, which matter when the service valves sit close to a wall and a straight hose will not turn.
Best for: A unit mounted tight to a wall

A full gallon of solution rather than a sample, which is roughly what one thorough flush of a badly scaled unit consumes.
Best for: A unit that has never been descaled

The hardware without the chemistry, for anyone planning to flush with five percent white vinegar, which is the traditional and entirely valid method.
Best for: Flushing with plain white vinegar

From a pump manufacturer rather than a kit assembler, so the pump is the part of the kit that was actually specified.
Best for: Anyone who will reuse the pump elsewhere

The cheapest complete kit here. A sixth horsepower is enough circulation for a residential heat exchanger.
Best for: A budget annual service

These results are planning figures from published test procedures, code tables and manufacturer specifications, and they do not replace a sizing done by the person who will sign the permit. A water heater is a pressure vessel: never cap, plug or remove the temperature and pressure relief valve, and replace one that will not open when the test lever is lifted. Gas line, flue and vent work belongs to a licensed plumber or gas fitter, because improper venting produces carbon monoxide. Water at 140 F scalds a child in seconds. Turn off the breaker before touching an element, and never energise an element that is not submerged.
